Ive learned a pile in the last few days.
Found the problem--was a bad connection at the black/red connector that goes into the combo switch. The wiring diagram does not match up exactly with what i have, so I wonder if switch was replaced previously.

Good news on the solution, Kent! I believe that there were colour-code variations over the production run.
