First in is the copper crush washer, then the headers, then the two spacers or collets which go round the header pipe and finally the cast iron collar with the two nits and washers that push/hold everything in place. Definitely the headers go in BEFORE the two semi circular spacers.

Yes, it was as I thought: I had the spacers in on top of the crush washer, and then the headers on top of that. I put the spacers Around the header pipe, reinstalled and we are good to go.
